Please contact your underwriter if you need assistance in completing these applications. Affordable housing applications can be accessed on the members insurance application page (login required).

Commercial Insurance Applications - Public Housing

These are the core insurance applications necessary to provide insurance coverage for Public Housing members. The Commercial Output Program is approved in some states. In non-approved states, use the Commercial Property Application. Please contact your underwriter if you need assistance in completing these applications.

Commercial Liability Insurance Lori Luponio Abstract: Use with Commercial Insurance Application when requesting coverage for Commercial Liability premises and operations exposures.     3. Commercial Property Application Lori Luponio Abstract: Use with Commercial Insurance Application when requesting coverage in non-COP approved states for your buildings or business personal property.     4. COP (Commerical Output Program) Application Lori Luponio Abstract: Use w/Commercial Insurance Application for buildings, business personal property, computers, mobile equipment, and income & expense. Includes options for crime & equipment breakdown and limited coverage for flood & earthquake (not available in all areas).     5. Commerical Auto Application Lori Luponio Abstract: Use with Commercial Insurance Application when requesting coverage for auto liability and physical damage including hired and non-owned auto liability. Underwriter will provide a separate state supplement if required.
