Housing Authority Insurance Group Affordable Housing and Public Housing insurance applications can now be downloaded, filled out, and saved to your computer using Adobe Acrobat Reader (you will not need to use the full version of Adobe Acrobat to save changes to the application). Once you have filled out and saved your application, it can be submitted via e-mail. You can also fill it out and send it in by fax or mail if you prefer.

Using HAI Group Application Forms

• Open the application you would like to fill out;
• Enter the information you need into the application (fields  to be filled out are highlighted);
• Select the "Save As" button to save your completed application;
• Select the "Submit" button to send it to HAI Group via e-mail; and
• Select the "Print" button to print it.
